If you no longer have the email that FAROS sent to you, you can check in FAROS. Look under self service > service information > request history. This question requires a valid date format of MM/DD/YYYY.

Allow at least 5 business days for User Administration to complete your FAROS request. If you still haven't received an email from FAROS informing you that your request has been processed, you can then continue to fill out this form and we will investigate.
