Common examples: Site Supervisor - Mid Term Evaluation, Faculty Mid-Term Evaluation, Supervision Site Review, etc.

For each uploaded evaluation, please provide the following information.

Evaluation Name

Submitter(s): Which user submits the form in Supervision Assist (trainee, faculty, supervisor, coordinator)
Assessing: Who is this form assessing? (self-evaluation, trainee, faculty, supervisor, supervision site, program)
Reviewers: Who is able to see the results of the completed evaluation/form?(trainee, faculty, supervisor, coordinator)
Notified: Should anyone be sent a notification when the evaluation/form is completed? (trainee, faculty, supervisor, coordinator)
