Title V Public Input Survey FY20

Virginia Maternal and Child Health Public Input Survey 2019

What is the Title V Maternal and Child Health (MCH) Block Grant: The Title V MCH Block Grant is a Federal-State partnership program that provides funding to ensure vital services for mothers, women, children and youth, including children and youth with special health care needs and their families. In Virginia, the Title V program is managed by the Virginia Department of Health (VDH) Office of Family Health Services. Allocation of Title V funds is based on Virginia’s maternal, infant and child health population, priorities, and needs. Virginia Block Grant funds support state, regional and local programs as well as staff. The Title V MCH Block Grant is Virginia’s dedicated source to support core MCH public health functions. Title V is critical to ensuring the health and safety of our state’s mothers, infants and children.

Purpose: The VDH Office of Family Health Services seeks your input concerning Virginia’s Title V MCH priorities, needs, and proposed program activities. Every year as part of the annual MCH Block Grant application, Virginia seeks feedback from key partners, stakeholders, and families. Your input is greatly valued, and we appreciate your completing this survey to help us set priorities and improve the health of some of our most vulnerable citizens. Please review Virginia's Title V Block Grant and then provide your comments. Your participation is voluntary and responses are kept confidential.

The survey will take approximately 12 minutes to complete.
