Survey and Support service information

This survey is one of the ways we are trying to find out what young people think about being in different types of out-of-home care. We are also organising face to face meetings with young people. The questions in the survey have been written with young people who have a care experience. We are interested in knowing about kinship care, foster care, residential care and lead tenant.

We have grouped the questions into different sections:

- Health and Life skills
- Culture, Community and Relationships
- Education and Employment
- Placement
- DHHS/Child Protection and Case Planning
- Court
- Your Voice

The sections will appear at the bottom of the second survey page. You can choose which sections and questions you answer - you don't have to answer every section - It's up to you!

You might find some of the questions make you think about times or events that are upsetting. The following organisations can help you if you want to talk about that:

Lifeline Australia
What is it: Lifeline is a national charity providing all Australians experiencing a personal crisis with access to 24 hour crisis support and suicide prevention services.

eHeadspace
What they do:
Free and confidential phone and online service for young people 12-25 years. Qualified youth mental health professionals provide support to young people worried about their mental health or experiencing issues such as depression, bullying and isolation.

Frontyard Youth Services
What they do: If you are a young person aged 12 to 25 years who is experiencing homelessness, is at risk of homelessness, is disengaged or requiring support, Frontyard can support to make decisions about your future.
Available Monday - Friday 9am - 8pm, Saturday and Sunday 10am-6pm

Privacy and Confidentiality
Before you start the survey you need to read this section and check the box to say that you have understood what it says.

Any information that you give us is confidential and anonymous. That means that we won’t record the details of who you are, including your name or where you live and anything you say that might identify you to other people won’t be included in our final report or published anywhere by us.

If you say anything about someone harming you or doing the wrong thing or breaking the law in this survey, we will have to report that to the police along with any other information you have included that might help the police find out more about what happened. Because we won’t know that it was you, we won’t be able to follow up with you. The organisations listed above will be able to advise and support you to decide what to do about those kinds of experiences.

You don't have to say anything that you don’t want to say. Ever. You can decide not to continue with the survey at any time.

You should contact us if you:

- are worried about the process including how we have collected, used or stored your information
- if you have a concern about your experience during the consultation with the Commission, or
- if you want to make a complaint

We will use the information you give us to help create a report for the inquiry. We will give this report with clear recommendations, to the government to help improve out-of-home care.
