Transitions - Rating Exercise

Thank you for your interest in Health Quality Ontario's Transitions in Care project!

Health Quality Ontario is looking to better understand the experiences of patients who are leaving the hospital to recover at home (home also includes supportive housing, shelters, long-term care facilities and nursing homes).

In the spring of 2018, we gathered input from patients, families, and caregivers — all those who have experience with an admission to an Ontario hospital overnight. We heard from over 1200 people, who submitted 2700 ideas. We have summarized that input into 52 different Statements highlighting different aspects of the transitions experience.

Now, we want to hear from you about what is most important: Which of these 52 aspects are most important for you and which should be a priority for the health care system as a whole?
