Initiate Coaching

We are looking forward to working with your employee! Please complete the form below to initiate coaching for an employee. Upon clicking the "Initiate Coaching" button, the following will happen:

Scheduling First Session. Your employee will receive a welcome email with information on how to schedule their first session. The employee is responsible for scheduling their first session. If we have not heard from them within 3 business days on this initiation form being submitted, the employee, their supervisor, and you will receive a reminder.

The supervisor will receive an email with what they can expect from the process with regard to updates, input, etc. After the employee schedules their first session, CareProfiler will reach out to the supervisor to schedule an initial consultation session.
