1. Welcome to the Nashville Community Health + Well-being Survey! We’re glad you are here.

This survey is being conducted by the Survey Research Laboratory at the University of Illinois at Chicago on behalf of the Metro Public Health Department (MPHD) of Davidson County and NashvilleHealth.

Your household has been randomly selected to participate in this survey which aims to learn about the health of residents of our county. Your answers will help inform and guide health and wellness policies, practices and activities in our community by organizations such as the health department, hospitals, non-profits and businesses. This survey is part of our Community Health Assessment; complete information on this assessment is available at www.healthynashville.org. Survey results will be available at the same site in early 2019.

We would like to ask the adult (age 18 or older) in your household who will be the next to celebrate his or her birthday to answer the online questionnaire.

Please be assured that all of your answers will be kept strictly confidential. Participation is voluntary and you may skip any question that you do not wish to answer.

The survey should take about 20 minutes to complete. We’ve optimized the questionnaire for easy use with mobile devices, in addition to laptops and other computers.
