Maternal and Child Health Needs Assessment Survey

Purpose

All states participating in the Title V program of the Social Security Act are required to conduct a statewide Maternal and Child Health needs assessment. The Virginia Department of Health (VDH) invites your participation in this vital activity. The goal of this survey is to garner stakeholder input in selecting priorities for maternal and child health programs in the Commonwealth.

Maternal and child health refers to the health and services (e.g., primary care, preventive care, reproductive health, community resources and programs) of women, pregnant women and their partners, infants, children, adolescents, children and youth with special health care needs, and the LGBTQ+ population. Maternal and child health are influenced by social and economic factors such as family, education, income and behavioral factors. Programs and resources that are considered maternal and child health include but are not limited to: safe sleep classes and resources, injury and violence prevention, housing and social support programs and services, early childhood education programs, genetic counseling, oral health and smoking cessation. When completing this survey, we ask that you think broadly. Maternal and child health is not limited to pregnant mothers and infants.

Please take a few short moments (approximately 5-10 minutes) to complete the following survey. You are welcome to delegate completion of the survey to the surrogate of your choice, as well as forward the survey link to additional friends and colleagues to complete. If a paper copy of the survey will be more convenient for you or your organization, please contact the VDH Division of Population Health Data at population.health@vdh.virginia.gov.

We ask that you assist us in meeting our federal requirements and complete this survey by January 15, 2019

Thanks, in advance, for your participation.
