We can only solve problems we know about! The FHC appreciates your time and attention to detail filling out this maintenance request form. We will address your issue as quickly as possible. Please keep in mind that it may take a few days depending on the type of maintenance required.

A copy of your request will be sent directly to your Director of Property, House Director (if applicable) and Regional Property Manager

This form is NOT for emergency maintenance (ex. broken pipe). Please contact your Director of Property or House Director (if applicable) directly if this is an emergency. 

IMPORTANT NOTE: This Maintenance Request Form is only intended for chapters who are part of the FHC's pilot program. If your chapter is not listed in the drop down menu on the next page then your maintenance request should be communicated directly to your Director of Property.
