Candidates interested in applying for a position on the Constituent Advisory Committee must submit a complete bio-data form and upload the required supplemental materials within it. Please submit your completed form with supplemental materials by April 25, 2021.
It is a good idea to make sure have all the supplemental information available for upload before you begin your bio-data form. The following supplemental information is required within this form:
1. Copy of current dental hygiene license.
2. Within this form, you will be asked to describe why you feel your experience and skill set would benefit this position in 500 words or less.
3. Copy of your resume
