Regional Volunteer Award Nomination

Thank you for taking the time to recognize a key volunteer! All nominations are due by April 30, 2019. Please keep the following guidelines in mind when completing your nomination.

• The award is for volunteer service with the National Psoriasis Foundation completed July 2018 to present
• Members of the Board of Directors and the Medical Board are ineligible to receive this award for their service in these capacities.
• Please limit nomination to activities completed on behalf of the National Psoriasis Foundation.
