YouTHrive Screening Survey

Thank you for your interest in the YouTHrive study! The purpose of this study is to test out the YouTHrive program. The goal of the YouTHrive program is to provide support to young people living with HIV by providing information about ways to deal with some of the challenges of living with HIV, such as taking medication.

We are currently looking for young people living with HIV who are willing to join the YouTHrive study. If you are eligible, we would ask you to attend an in-person visit. If you are eligible for the study, we will ask you to attend an in-person visit 5 and 11 months later. We will also ask you to complete an online survey in 8 months. Participants will receive up to $170 for completing all parts of the study.

Before you can participate, we must determine whether you are eligible with a short screening process which will take approximately 5 minutes. The screening process is optional, and you may choose at any time not to participate. If you choose to proceed, you will be asked a few questions today. Your answers will determine whether you are eligible to participate in a larger research study. You can decide to stop answering questions at any time. If you don't answer all the questions, we may not be able to determine if you are eligible for the study.

If you are eligible for this study and are interested in participating, the screener will ask you for your contact information to schedule a visit for you to meet with study staff. Just so you know, any information shared in this survey is strictly confidential and will only be used for research study purposes. All the information that is gathered from you is safely stored. If you are not eligible for the study, you can choose to have your name and contact information kept by the study team if you would like to be contacted about future research studies. Your information will be kept on a secure computer server. If you choose not to be contacted for future studies, your information will be deleted. If you are eligible and choose to participate in the study, you will be asked similar questions at the enrollment visit to verify your eligibility.

We also want you to know the potential risks of screening for the study. These include the risk of breach of confidentiality. Data is encrypted on this survey website to minimize this risk. When study staff receive your information, it will be kept on a secure computer server. Therefore, we do not believe that the risk of a breach of confidentiality is high. You may not benefit personally from being in this research study.
