If you are under 16 you will need to ask permission from your parent to fill out this form. Our preference is for anyone getting baptised to be at least 12 years old. If you would like to discuss this further please contact the church office on 02 99728688.

7. On the day of your baptism a photographer will take photos for you to keep of your baptism moment. Sometimes we use these images for social media purposes. Please indicate if you agree for us to use your image (or your child's if under 16) *This question is required.
