Ms. Doe currently has a home health episode due to a recent BKA.  The home health nurse just found out that Ms. Doe attended her grandson’s college graduation ceremony. Ms. Doe is no longer homebound therefore does not qualify for home health services and should be discharged.

A physician can submit for reimbursement from Medicare when medications are being discussed with a home health nurse via telephone, but not when talking to the patient via telephone even when changes in medication need to occur.

Nurse Practitioners can perform the face to face encounter, but may not certify for Home Health.

In addition to the face to face attestation, documentation related        to Home Health qualification is required in the patient medical record on the day of the face to face.
