AFO Course Material Request Form

This material request form is intended for AFO Course hosts that are contracting directly with an AFO Instructor. If you are contracting with NRPA to provide an AFO Instructor, you must refer to the Contracted Course material request form here, https://www.nrpa.org/certification/AFO/AFO-Course-Hosting/.

Orders must be received at least 10 business days prior to the course date or additional fees will apply. Any additional fees acquired for late requests will be added to your invoice (i.e. rush shipping fees).

Refunds will be issued only for unused, unopened course manuals. Course hosts are encouraged to order additional material in the event of last minute registrants.

(Packet fee includes: AFO manual, silde rule, handouts, and practice quiz)
* Shipping for Manuals: Standard shipping orders will include a $4 handling fee, 2-DAY shipping orders will include a $4 handling fee plus shipping and Next-Day shipping orders will include a $20 handling fee plus shipping.

7. PO#

ALL exam booklets, electronic score roster with participant emails and answer sheets must be returned to NRPA within 10 days following the course. Upon receipt, you will then be invoiced for used exams only.
