Climate Challenge Entry Form

Entries are due by midnight on March 31, 2020.

BEFORE YOU START THIS ENTRY FORM, be sure to have with you:

The YouTube link to your video.
The contact info for all the film makers. (Name, grade, school, and email if available for all of them, plus a mailing address and phone number for one of them.) Please provide an email address that someone from outside your school can send mail to. You may have between one and five film makers on a video.
Name and email for your teacher IF you completed the video as part of a class.
Scan or photo of completed "Consent, Waiver of Liability, and Release" form.
Scans or photos of completed "Right to Use Video Likeness" form(s), ONLY IF you have video or photos of identifiable people in your film.

3. Mailing Address of Film Maker #1 (Be sure to include the entire address.) *This question is required.

4. Phone Number of Film Maker #1 (Be sure to include the area code.)

5. Grade of Film Maker #1 *This question is required. This question requires a valid number format.

6. School of Film Maker #1 (Include the name of the school you attend, or write "homeschool" if that's the case) *This question is required.

7. If your film was created as part of a class, please provide the name of your teacher.

8. If your film was created as part of a class, please provide your teacher's email address. This question requires a valid email address.
