Read each characteristic and choose the response that best describes your clinic's current practice of the characteristic. (Refer to the
Guide to Youth-Friendly Services Assessment for further explanation.)
- Select “Yes” if your clinic follows the practice and does not need improvement in that area.
- Select “No” if your clinic does not follow the practice in any capacity.
- Select “Needs Improvement” if your clinic partially follows the practice, but it could be improved, or if you want more information about your current practices.
Your total score for each section will appear on your Assessment Report. Your Assessment Report and instructions for next steps will be sent to you within a few days.
