All prospective and returning organizations must fill out the 2020 PACT Application located on the Park District Website.
PACT Applications require an annual fee of $15 for organizations based in Oak Park and $25 for outside organizations. Please send your check to Park District of Oak Park, 415 Lake Street, Oak Park, IL 60302 or call 708/725-2000 with your credit card information by the October 4, 2019 deadline
