Youth Opioid Prevention Needs Assessment Survey

Introduction

This survey is designed to help us develop a toolkit to guide schools, parents, students, and community leaders in preventing opioid use among youth ages 15 to 19. We want to learn more from school and community-based organizational staff, like yourself, to see what we need to include in the toolkit to be effective, relevant and successful in your community. This survey will ask you questions about:

•    What you know about opioids;
•    How you are talking with youth about opioid use;
•    The risk factors you see among youth that may lead to opioid use; and
•    What topics you would like to see included in a youth-focused opioid prevention toolkit.

This survey is being led by Health Promotion Council in partnership with Public Health Management Corporation and is funded by the Pennsylvania Commission on Crime and Delinquency. This survey should take about 10 minutes to complete. Your survey participation is voluntary and you can quit the survey at any time. You will not be asked for your name in this survey. Please do not share any information that could identify you personally or someone that you know. Survey responses will be used to develop a youth-focused opioid prevention toolkit for use in educational and community settings. Any briefs, white papers, presentations or peer-reviewed publications that result from this survey will not include your personal information.
