Enrollment information

Dear Parents or Guardians,

BEFORE YOU BEGIN, IT IS RECOMMENDED THAT YOU COMPLETE THIS ONLINE ENROLLMENT FORM ON A LAPTOP OR COMPUTER FOR EASE OF READING. ALSO, PLEASE ENSURE THAT ALL NAMES, EMAIL ADDRESSES, AND CELLPHONE NUMBERS ARE ACCURATE.

FOR NEW STUDENTS, we require a report card or Form 138 (original and photocopy), birth certificate (original and photocopy), two pieces of 2x2 colored photo, Certificate of Good Moral Character from previous school, admission slip (our registrar), and a completed enrollment form. To speed up this enrollment process, please scan these documents and then you can upload these at the end of this online enrollment form or you can bring these to the Registrar. This enrollment process will be considered complete after these required documents have been submitted and after you have paid the required tuition and fees. This entire enrollment process could be completed at www.jjscastleson.org.

Assessments may be required of new special education students and fee is paid separately.

Entrance Exams for new students are no longer required. However, the new student is required to take an assessment test on this link https://www.jjscastleson.org/teachinglearningcommunity
