READINESS ASSESSMENT: Your Information
The purpose of the Readiness Assessment Survey is to help your medical office and hospital team to determine levels of readiness related to the education process and the required elements of Chronic Care Management or Care Coordination in general. The survey will ask you to evaluate six major focus areas that are needed to be successful with care coordination processes at various levels within your organization.
This is meant to serve as a tool to help the HomeTown Health team to identify readiness for implementation of various care management areas and what would benefit your organization the most. It will help your team to identify areas you may want to focus on to provide successful care management services.
For organizations that have already implemented CCM, this tool will help you to identify areas you may want to improve upon to ensure compliance and to help support a successful program.
We'll also ask for some information on your preferences for the SHIP Grant Care Coordination Project this year to better serve you!
