Community Health Needs Assessment
The Pride Center of Vermont is collaborating with the You First & Tobacco Cessation programs at the Vermont Department of Health to provide a Community Health Needs Assessment (CHNA). The goal of the CHNA is to improve the health of LGBTQIA+ Vermonters. The main question we want to explore is: how does being LGBTQIA+ influence our physical, mental, and / or social health? By participating in this survey, you bring us one step closer to understanding how the experience of being LGBTQIA+ in Vermont relates to all aspects of our collective health and well-being.

This survey is designed for all people who identify as lesbian, gay, bisexual, transgender, queer, questioning, intersex, asexual, aromantic, pansexual, two-spirit, HIV-positive, or another sexual or gender minority.

All survey answers will be anonymous. We understand that information about your life is personal. It is our top priority to keep the information you share private and secure. All communications between you and the survey are encrypted, and our data centers are physically and digitally secured.

We also know that your time is valuable. That is why we try to keep the questionnaire to 20 minutes or less. Depending on the details of your health history, some participants may be asked more questions than others. All questions are optional, but with more information provided we are able to have a clearer picture of the health status of LGBTQIA+ Vermonters. We are also offering a raffle drawing of four (4) $50 gift cards to a store / restaurant of your choice, and eight (8) $25 gift cards to a store / restaurant of your choice. All participants who choose to enter the raffle drawing will be re-directed to a separate page following the survey to maintain participant anonymity.
