2020 Warriors to Lourdes Application
APPLICATION INSTRUCTIONS
Thank you for your interest in attending the 2020 Warriors to Lourdes Pilgrimage. All applicants should complete the following application by the application deadline, January 15, 2020. In addition to this application, applicants must also submit a Chaplain endorsement letter, the appropriate medical form(s), and payment (if applicable) by the application deadline. Applicants who do not submit all of these materials by the application deadline will only be considered once all applicants who have complete applications have been considered.
Please note that each individual wishing to attend this trip must complete an application. This includes all designated caregivers, adult companions and child companions traveling with a wounded warrior or warrior pilgrims. Applicants traveling with caregivers or companions will be considered together in the selection process.
This application may be started, saved, and finished at a later date. Once you enter your email address, if you exit the application page, you will receive an email with a link to complete your application. Please use this link to complete (or edit) your application, rather than start a new application.
Applicants will be notified of their selection status beginning February 18, 2020.
