Happiness Project Groups Survey

Thanks so much for your willingness to take this survey.

I'm excited to jump in and play a more active role with the Happiness Project groups, and I'd like to start by asking a few questions. Feel free to offer as much or as little information as you want.

Once I have your answers, I'll post the full list of where all the groups meet (on my blog and Facebook Page). I'll also provide public contact information, for the groups who choose to provide it in Question #6.
