In order to comply with federal regulations, the following evaluation form must be completed for each trainee or intern sponsored by the French-American Chamber of Commerce (FACC) on a J-1 visa at your company.
The evaluation must be completed in full and signed by the exchange visitor's direct supervisor. Please note that failure to complete this form will affect your ability to host future exchange visitors and future sponsorship by the FACC.
