What documentation will I need?
As well as information on your proposed nurse-delivered (team-based) model, you will need to provide:
- Basic contact information
- Incorporation status of the primary health care organisation (Information regarding type of legal entity, GST, ABN)
- Accreditation details
- Name of clinical software in use
- Key contact details
The following documents are mandatory and will need to be uploaded with your completed application:

- Letters of support from external parties
How do I go back?
- Use the 'Back' button at the bottom of the page.
- Do not click the 'Back' button on your browser. This may cause your progress to be lost.
- If this happens, click 'Refresh'. Depending on your browser it may have saved your information, but there is no guarantee.
How do I save my application?
- Click 'save and continue later' in the grey box at the top of the page.
- It will ask you to input an email address to send your application.
- Check your spam / junk folder if the email does not appear in your inbox.
- Click the link in the email to continue your application.
How do I upload documents if I don’t have a scanner?
- You can use a smartphone to take a photo of the document. Make sure the words are legible (when zoomed in).
- Email the photo to yourself.
- Download the photo from the email and save in an easily accessible place to upload to the application.
What does it mean if after I click 'Next' my page says 'There was an error on your page. Please correct any required fields and submit again.'?
- This means you haven't filled in the 'required fields'.
- These boxes have a red asterisk * next to them.
- These questions must be completed before proceeding any further with the application. It will show you which ones are missing.
Do I get to keep a copy of my application?
- In the final stages of the application you will see a 'Review my Document' page.
- This allows you to review your application before clicking 'Submit'.
- Click 'Download PDF Version' at the end of the page to download a PDF copy of your application. This may take a few minutes to download.
How do I know if my application has been submitted?
- Once you are satisfied with your application, click 'Submit' to send your application to APNA.
- Your application is automatically sent to APNA.
- You will receive a confirmation email that your application has been sent.
