The following questions are designed to enable us to undertake an initial scoping of the size and needs of your hospital or healthcare setting. We will also seek to identify the key personnel with whom we can collaborate to support the successful implementation of iCOPE within your healthcare setting.
The survey is not expected to take more than ten minutes of your time.
If you need more time or wish to return to the survey at a later stage, simply click the TOP RIGHT corner of your screen, you will then be prompted to provide an email address where your personal link will be sent to complete your survey at a later stage.
You will also be provided with a copy of your survey responses upon submission, and this will be emailed to you for your records.
