We are exploring opportunities to better support our community through the Fort Collins Utilities Affordability Programs. In order to ensure that we continue to meet the needs of all current participants, we are asking for your help in providing the following information.
Please note, Utilities typically does not ask for proof of household size or income to accompany the MAP application. However, we do ask for your signature on an affidavit that states you will provide copies of financial records, if asked. You do not need to provide copies of financial records with this request. We are simply asking for this information to better understand how program changes might affect current participants.
