SEIU-WEST WORKLOAD TRACKING FORM (Revised Feb. 2020)

To help resolve workload issues in our workplaces, SEIU-West has created the following Workload Tracking Form (WTF). It takes about 5 minutes to complete.

Submitting your WTF online helps us to address your workplace's workload issues more quickly.
You don't have to give your name, and no information that could identify you will be shared with the employer.
We encourage you to send us a WTF for each shift during which a workload-related issue arises in your workplace/unit.
