Private School Application 2020-2021

Introduction to Private School Application

Thank you for starting the Private School Application for the 2020-2021 school year. This form is for both new and renewing schools for all necessary reporting to the State Board of Education. This application includes enrollment and staffing reporting that was previously collected in the Educational Data System (EDS). Note that there are other reporting requirements to the Office of Superintendent of Public Instruction and immunization reporting for the Department of Health. Before you begin, we suggest previewing the application. If you have all of your information together, this application will take about one to two hours to complete.  Partial applications that have not been submitted will not be considered.

Save function: There is a save function. If you want to use the save function, you will need to create a login account with SurveyGizmo. State Board of Education staff cannot help with questions about the save function or retrieving your username/password. Please refer to SurveyGizmo's help page for assistance with issues related to the software.

Enrollment and staffing report: In an effort to streamline processing of enrollment and staffing information, the State Board of Education is collecting data from renewing schools on October 1, 2019 enrollment information and 2019-2020 staffing information during the application process for the 2020-2021 school year. New schools will be asked to report prospective enrollment and staffing information for the 2020-2021 school year. The reporting fields will look nearly identical to the prior reporting fields in the P105B application. The differences are that the enrollment reporting tables will include race/ethnicity without gender and there will be a separate question regarding enrollment by gender.

Health and fire inspections: In this 2020-2021 school year application, only schools that are new, moving location, or have undergone a major renovation will need to submit documentation of fire and health inspections. For the schools that this applies to, please have that documentation ready to upload. The rest of the schools will be able to self-certify in the application process. In some counties, the health inspection may be called "school safety inspection" or other names depending on local policy.
 
Guidance on identifying a county health inspector: Identifying an appropriate health inspection organization is straightforward. Each county has a health department. Either contact the county health department directly or contact the building code personnel at the county/city to be directed to the health department. Please keep in mind that although identifying the appropriate health inspector is straightforward, local health codes may differ from area to area.

Guidance on identifying a licensed fire inspector or an appropriate governmental fire inspection organization: The type and availability of fire inspectors differs throughout the state. Due to the variance across the state, a good start would be to contact the building code personnel at the county/city and ask to be directed to a licensed fire inspector or an appropriate governmental fire inspection organization based on the private school building's address. Options that may be available are a county, city, or other licensed fire inspector.

Note: Retain documentation of health and fire inspections in the school's main office.

Certificate of compliance upload: At the end of the application, you will be asked to upload the Certificate of Compliance Signature Page that can be downloaded then signed and scanned for upload. By signing this document, you certify that your private school is in compliance with all requirements.

Additional campus info: Schools with additional campuses will be able to still apply with one application. The parent campus should fill out one private school application that lists all additional campuses (questions will prompt you to list additional campuses). However, to ensure that individual school buildings are being correctly identified, State Board staff have assigned unique building codes to each of the additional campuses. Please use the EDS directory information to look up those building codes if you have additional campuses. If you did not previously identify additional campuses, please do during this application and building codes will be assigned to the additional campuses after the application has been processed.
  1. The school code can be found in OSPI EDS Directory Information.
  2. Use the "Organization Category" dropdown box to select Private School.
  3. Use the "Organization Name" text field to enter the name of the school, then the result of that search will populate in the lower part of that page.
  4. On the left-hand side of the search results, you will see the name of the school followed by the school code in parentheses.
  5. Enter the school code found in the parentheses.

Certificates of approval: Certificates of approval will be posted to the State Board of Education private schools website as soon as possible after approval. These will be posted as a bundle of certificates for approved schools. Once posted, please scroll through to find your school. This document demonstrates to the public that you are a private school approved by the State of Washington.
