7. I hereby request to stay in the residence halls later than the deadline on May 16, 2020 at noon. I understand that no request for stay past May 17, 2020 at noon will be approved.

By signing this form I accept and will abide by all terms of the 2019-2020 Residence Hall and Dining Services Agreement through my stay in the residence halls. If approved to extend my agreement, I agree to pay the additional pro-rated portion of my room charge which will be charged to my student account.
