The workshop report from the NIST-FEMA Workshops on Post-Earthquake Functional Recovery occurring between 1/27-2/6/2020 will include a list of participating organizations. The purpose of this form is to allow participants to opt-in to having their name printed along with their organization as a workshop attendee. This workshop report will be made publicly available.

Regardless of opting-in or –out, no statements will be directly attributed to any single individual or organization. Please note that by not submitting your name to this survey you are opting to have your organization listed as a workshop participant without your name.
