New York City COVID-19 Stigma Incident Survey

We are asking New Yorkers to share incidents of stigma that are occurring in your communities related to the Novel Coronavirus (COVID-19).

We know that COVID-19 can negatively impact our emotional well-being, not just our physical health. In order for the NYC Health Department to better protect and promote the health of all New Yorkers, we're asking you to share stressful stigma incidents with us.

There are many social media posts and news reports that are not rooted in science. This content is offensive, demeaning, and, more often than not, racist. While it's important that everybody separates facts from fear, stigma remains a daily reality for many New Yorkers.
Submissions are not considered to be an official report to the City of New York.
As the Health Department, we have limitations in responding to your submission(s). As such, your submission(s) will not receive a response from any NYC Health Department staff, nor will your submission be forwarded to other City agencies for a response. The NYC Health Department does not have the authority to intervene in cases of discrimination. Listed below are resources you may contact for further assistance in the case you need to report a crime or discrimination.

Your submission, however, will assist us in understanding what kinds of stigma events are occurring, how they are negatively impacting the health of your community and how we may be able to develop improved messaging to help combat fear, hostility and misinformation.

We do not collect any identifying information (for example, name, address, workplace, etc.)
DO NOT share the names of individuals who have experienced stigma nor the names of aggressors of stigma.
