Women's Health Survey

Gynecological conditions, such as fibroids, endometriosis, and adenomyosis, are so prevalent, yet there is still much to learn, especially about the way women seek treatment for their symptoms. We are embarking on a new research study to further explore patient decision making behaviors and racial disparities in gynecological care. We would like to hear from you about your experiences with these conditions. We plan to share this important information with both the medical community and with women like you.

This short survey will take about 10 minutes to complete. Your participation in the survey is completely voluntary and all of your responses will be kept confidential. If at any point you do not feel comfortable completing the survey, you may exit the survey and your responses will not be recorded.

IMPORTANT
For the best experience, complete this survey on a laptop/desktop computer. 

If you chose to take a short break while taking the survey, there is a bar at the top to save your progress. **
If you do not fill this out before you leave, you will not be able to return to the survey.** We recommend taking the survey in one session. 

Please make sure to enter your email address at the end of the survey. If you were notified of the survey from an email list, make sure that you provide the same email address that you provided for that list.  

There will be a few questions unrelated to the survey. These are for us to gauge the level of engagement from our survey respondents. Please answer these questions appropriately.
