This survey will remain open for as long as COVID-19 pandemic continues to significantly impact the livelihood of creatives in Indiana. It's being monitored in real time, so please submit it again if your circumstances change.

This information helps the Indiana Arts Commission (IAC) plan upcoming webinars and non-cash support for individual artists. It does not guarantee any specific action from the Indiana Arts Commission or the State of Indiana.

We may share the aggregated, anonymous results with other supporters of artists to help inform their efforts as well.
