Altarum, an Ann Arbor based nonprofit, is conducting this 5-minute survey to gather information about ambulatory care clinics’ top concerns, challenges, and support needs during this unprecedented public health emergency.
By providing your anonymous feedback you are doing your part to help illuminate the needs of ambulatory clinics and providers across the state so that information can be shared with state and federal agencies directing response efforts.
