While your normal operation is closed, are you continuing to serve meals to enrolled children or children in extended day childcare for first responder families? *This question is required.
Please select the CACFP waivers your organization has utilized during the COVID-19 outbreak. *This question is required.
Have you submitted a Meal Distribution Plan to your program specialist? *This question is required.
To take part in these waivers, you must submit a Meal Distribution Plan to your program specialist. You will receive an email with a link to the form.
