Teen Sleep Habits Survey During COVID-19 Pandemic

We would like to find out information about the sleep habits of students in grades 6-12. This will help us understand how teens have been sleeping during a time when many schools are closed and instruction is happening by many different methods (or not at all.) 

All of the responses you give will be anonymous and untraceable to you or your device. Please be as accurate as you possibly can. This will take about 12 minutes of your time.
