Volunteer Reference

Make-A-Wish Wisconsin Volunteer Reference

At Make-A-Wish, we create life-changing wishes for children with critical illnesses. Wishes are more than just a nice thing - a wish experience can be a game-changer for a child with a critical illness. The individual that sent this to you has applied to become a volunteer for the Make-A-Wish® Wisconsin and named you as a reference.  Our policy requires a reference check to determine a volunteer applicant’s character and likely success in a volunteer capacity.

Make-A-Wish® volunteers are utilized in a number of capacities, which may include working directly with the children we serve, fundraising and general administrative support.  The Foundation is seeking individuals who model reliability, responsibility, cooperation, attention to detail and respect for others.

Due to the sensitivity of working with families in crisis, it is crucial to understand the capabilities and qualifications of each volunteer applicant.  We encourage your candid responses to this person’s suitability as a Make-A-Wish volunteer. Any information you provide will be held in strict confidence and will be used for no purpose other than the determination of the applicant’s ability to volunteer.

Please answer the following questions and submit your responses as soon as you can.  References must be returned within 30 days. We appreciate your prompt cooperation in this very important process.
