Lead-Safe Homes Program Company Interest Form

The Lead-Safe Homes Program is looking for state-certified lead companies who are interested in doing lead risk assessments or lead abatement work on enrolled properties (enrolled homes are located all over the state). The program covers all or almost all costs associated with making homes lead safe:
  • The cost to conduct a lead risk assessment on the property.
  • Any costs for lead abatement work (with the exception of replacing lead plumbing in a home).
  • Some costs for minor rehab work, when that work is necessary to ensure the integrity of the lead abatement work.
  • Costs for training and certifying contractors' crews.
  • Travel costs for contractors when they must travel more than 50 miles to a jobsite.
  • The cost of pollution insurance for contractors.
  • The cost to conduct a lead-safe clearance on the property after work is completed.
Please complete this form to indicate your company's interest in having the Wisconsin Department of Health Services (DHS) inform you of projects in your service area.
