Effective October 21, 2020, all Michigan Adult Foster Care (AFC) and Homes for the Aged (HFA) facilities with bed capacity of 13 or greater are required to transition to a new platform for COVID-19 reporting.
On October 28, 2020, all licensed and operating AFC and HFA facilities with bed capacity greater than 13 must register in a Qualtrics reporting tool.

Upon completion of the initial registration survey, each registered location will receive an individualized link to access a future weekly reporting survey within the Qualtrics reporting tool.
The first weekly reporting survey must be completed on Wednesday, November 4, 2020 by not later than 12:00 PM (noon), and on Wednesdays each week thereafter.
At this time, Adult Foster Care and Home for the Aged facilities with bed capacity less than 13 are exempt from MDHHS weekly COVID-19 data reporting requirements.
