This Survey is open to every Colorado restaurant, bar, cafe and brewery owner or authorized representative.
There are 34 questions, largely multiple choice, that take about 8 minutes to complete. 
Responses are anonymous even if you provide your name and email, which will be used only to contact you by request or to send you the Survey Report.    

Survey results are intended to inform relief and recovery programs at the state and local levels. RESILIENT will use this data to structure programming and outreach. The survey report will be made publicly available.
