Community Testing Site Information Survey

Please respond to this brief survey with updates on new or existing community testing sites. The Department of Health Services' (DHS) COVID-19 Community Testing Map displays current COVID-19 community testing sites in Wisconsin and is an important resource for individuals to locate nearby community testing.   

This survey is for testing sites that are:

• Are open to the public regardless of provider affiliation or insurance status
• Meet all requirements for testing providers in Wisconsin

We will review survey responses and determine if new sites are eligible to be added to the Community Testing Map or will make changes to existing site listings.  We may reach out directly to sites to discuss any questions about the site or responses to the survey.
