OESIS Covid-19 PCR Testing Application

We are now able to offer next day results from major US Cities from the East Coast to Texas and 48 hour TAT from CA. These TATs are subject to coordination of pick up times and we will coordinate timing when you set up a meeting after completing this application and before paying the deposit below. We are able to offer a cash price for schools paying directly or an insurance covered hosting solution for testing at your school.

This application is for schools interested in the program and if it is complete you will receive a pdf copy by email. This application should be completed by a Head of School, principal, school chief financial or business officer with the authority to make this application.

8. Reliance for Opening School. Through our partners, we offer an RT-PCR Test with FDA EUA authorization. Please confirm the following understandings by responding below: *This question is required.
9. Sample Collection. OESIS will ship oral swabs to the school and these swabs are classified and registered as an FDA Class I exempt medical device ( Class 1 broadly means the safety profile equivalent to a bandage). The swab is for the taking of an oral pharyngeal sample from the back of the throat. Each sample container will be pseudo-anonymized (no personal patient information will be on it) when it is sent to the lab. The collection is straightforward and we will provide information and videos of how to do that. You will be responsible for the following:

1. Data & Privacy. Keeping a private inventory of the names of the students and faculty that correlate to the bar code numbers so that you or your medical provider or staff can then de-pseudo anonymize the results and then determine who might have tested positive.

2. Collection. We recommend and require that the school oversee the collection of swabs at school (not at home) as in a carpool line supervised and collected by a nurse or EMT or qualified athletic trainer within approximately a 6 hour period to ensure conformity. We will provide digital training materials.

10. Transport and Logistics. OESIS will coordinate all the logistics including the pick up of the samples and delivery to their labs. After an individual is swabbed, the swab needs to be maintained at a temperature between 36 and 46 degrees Fahrenheit.  The school's process should include placing the samples in a cooler or refrigerator until pick up.  Upon pickup, our couriers will keep samples on cool packs for the entire transit to the lab. We will provide a confirmed date within a week that you choose as well as a specific time for collection for ALL of the samples. The samples will be sent to our partner's labs depending on capacity, equally accredited for such testing. State or border closures and unforeseen abnormal transport circumstances may necessitate resampling with kits re-sent at no cost to you. OESIS cannot guarantee that the shipper will not fail to deliver the samples and we can not be held responsible for a failed shipment due to factors outside of our control (including closed borders or airports and the like). You will not be charged for cash testing if the shipment fails and you will be sent new swabs at no cost to you.

11. Insurance or Cash Price of Tests. The cash price of the test is $88 and that includes the test swab, the testing analysis and data transfer. Insurance coverage is also available. You may have to be a non-profit that is sales exempt to avoid sales tax in some states and you will need to provide such evidence before you are billed for cash testing. 11. Privacy & Disclosures. OESIS and its lab patner will have no information regarding who has been tested unless insurance reimbursement is sought, because you will only be providing numerical identifiers. In the event that you use the insurance route, our lab partner's billing provider will with your staff and families's consent utilize their information to seek reimbursement. The results of the testing will be provided in the case of insurance to a directing physician or appointed provider, and in the case of cash payment to a designated person at the school. Your school will determine a process to ensure compliance with all legal requirements including HIPAA and other privacy laws, and disclosures to the CDC or local health authorities. 12. Deposit for Cash Testing. Since you choose that you would like cash testing, we require a  program participation deposit of $5,000 per school payable immediately on approval and fully creditable towards confirmed cash testing orders, pre-ordering of swabs for future cash testing, or (for any unused portion) towards any OESIS services including conferences, PD, and memberships. This deposit has no application towards insurance testing as this is billed directly to the insurer with no financial responsibility for the school. *This question is required.
12. Insurance Testing. You chose your interest in applying for insurance covered hosting of testing for your communities. There is no cost to the school for insurance-covered lab testing as reimbursement is wholly through billing to the insurance providers of your students, families, and staff. Uninsured members may also be covered currently through Federal and State laws. Since you choose that you would like to use OESIS for insurance testing, you commit to working with us and our lab partners exclusively to set up and host insurance covered testing events for your communities for the next 12 months. *This question is required.
