We are now able to offer next day results from major US Cities from the East Coast to Texas and 48 hour TAT from CA. These TATs are subject to coordination of pick up times and we will coordinate timing when you set up a meeting after completing this application and before paying the deposit below. We are able to offer a cash price for schools paying directly or an insurance covered hosting solution for testing at your school.

This application is for schools interested in the program and if it is complete you will receive a pdf copy by email. This application should be completed by a Head of School, principal, school chief financial or business officer with the authority to make this application.

9. Sample Collection. OESIS will ship oral swabs to the school and these swabs are classified and registered as an FDA Class I exempt medical device ( Class 1 broadly means the safety profile equivalent to a bandage). The swab is for the taking of an oral pharyngeal sample from the back of the throat. Each sample container will be pseudo-anonymized (no personal patient information will be on it) when it is sent to the lab. The collection is straightforward and we will provide information and videos of how to do that. You will be responsible for the following:

1. Data & Privacy. Keeping a private inventory of the names of the students and faculty that correlate to the bar code numbers so that you or your medical provider or staff can then de-pseudo anonymize the results and then determine who might have tested positive.

2. Collection. We recommend and require that the school oversee the collection of swabs at school (not at home) as in a carpool line supervised and collected by a nurse or EMT or qualified athletic trainer within approximately a 6 hour period to ensure conformity. We will provide digital training materials.

11. Cash Price of Tests. The cash price of the test is $88 for OESIS member schools (check left-hand column of the link) plus transportation and that includes the test swab, the testing analysis, and data transfer: transport costs are extra and for a typical school can add $10 per sample. You may have to be a non-profit that is sales exempt to avoid sales tax in some states and you will need to provide such evidence before you are billed for cash testing. 12. Deposit for Cash Testing for NON-MEMBER schools. If you are not an OESIS member school (see left-hand column of the link) and would like cash testing, we require a  program participation deposit of $5,000 per school payable immediately on approval and fully creditable towards confirmed cash testing orders, pre-ordering of swabs for future cash testing, or (for any unused portion) towards any OESIS services including conferences, PD, and memberships. This deposit has no application towards insurance-covered testing as this is billed directly to the insurer with no financial responsibility for the school. If insurance does not cover testing for some or part of your community, you may need this. *This question is required.
