Parent Voice Survey 2020

School has been closed since March 13^th. This survey will help us understand what you and your children have been doing, how you have been feeling during this time, and what we can do to help when school opens again. There are no right or wrong answers, just your thoughts and feelings.

1. PLEASE READ THE FOLLOWING CAREFULLY
I understand that I am answering this survey voluntarily and nobody will know how I answered any of the questions.
I understand nobody in the school district will see my answers to any of the questions. The school will not even know if I take the survey.
I understand that there is no reward for answering the survey and there is no consequence if I do not answer any of the questions.
I understand that my name is not on this survey and there is no way anyone can know which survey is mine.
I also understand that some of the questions are about how I feel about my child being at home and doing her/his schoolwork at home.
I understand that some questions might me make think about things that make me feel unhappy.
At the end of the survey there is a phone web link and a phone number where I can get more information about COVID-19 and can talk to someone about my feelings if I want.
