In order to best tailor my business’s services to the needs of the community, I need a little bit of information about the type of written and oral communications, paper or digital, that are demanded of you at work, school, or in business. I may also ask questions about resource availability and COVID-19 influences to discern immediate needs and challenges. The number of questions in the survey may vary from person to person, averaging at 35. It should take about 15-20 minutes to complete.
Don’t worry! Unless you volunteer to disclose it, no personal information, such as your name, phone number, or email address, will be stored. If you choose to share your personal information, you will have an opportunity to opt into a drawing for a 25$ Amazon gift card if you complete the entire survey by August 10, 2020. This information will be used to reach out to you for an opportunity to participate in future surveys or focus groups. You will never receive advertisements from Cosmic SophisTree LLC because of the information you share through surveys. This survey is only limited to those who are 18 years old or older.
The personal information that you provide will be stored on the SurveyGizmo platform for 12 months before it is deleted. You may request personal information and survey responses that you provide to be deleted at any time. If you choose to withdraw or delete your personal information before the 12 months have ended, you will also be withdrawn from the gift card drawing. You may also edit or change the information you provide at any time.
Whether you choose to disclose your personal information or not, no information will be sold to any third party person or business. The information will only be used by and for Cosmic SophisTree LLC.
