Pandemic Awareness Committee (PAC) Membership Application

Butler School District 53 identified a need for a Superintendent Advisory Group in response to Policy 4:180 Pandemic Preparedness; Management; and Recovery and created the Pandemic Awareness Committee (PAC).

To apply to be a part of the Pandemic Awareness Committee (PAC), please complete the following 5 questions by Friday, October 2, 2020.

3. As a committee member, you must agree to the following statements. Please read all the statements below and place a checkmark next to each one to indicate agreement.

I understand the committee is an advisory committee and cannot make authoritative decisions.
I understand my membership on the committee is voluntary and I must attend all meetings. If I cannot attend a meeting, I will provide advanced notice.
I understand I will be removed and replaced if I miss more than 2 meetings without providing prior notice.
I have the ability to virtually attend meetings.
I agree to respect differences of opinion and refrain from attributing commentary to members of the committee outside of the meeting
