YMI COVID In Person Mentoring Waiver

All YMI Mentors are required to complete a self-assessment before arrival in the school EACH WEEK.  If a Mentor exhibits any of these symptoms they are NOT to mentor in person that day:

  • Temperature of 100.4 or greater
  • Sore throat
  • ​​​​​​​New uncontrollable cough that causes difficulty breathing
  • Diarrhea, vomiting, or abdominal pain
  • New onset of severe headache, especially with fever
  • Loss of taste or smell
If a mentor presents with symptoms of COVID-19 they would need to stay home for 10 days and be fever free for 24 hours prior to returning to in-person mentoring.  
YMI In-Person Mentoring Protocols:
 
  • Self-Screening Procedures – All YMI Mentors are required to complete a self-assessment before arrival in the school.
 
  • Stay Home If You Are Ill or awaiting results from a COVID test – YMI Mentors are not to report to their school(s) if they are sick, develop COVID-19 symptoms, or believe they may have been exposed by close contact. It is the expectation that YMI Mentors inform their Campus Coordinator (CC) as soon as possible if / when symptoms are presented to the Mentor. The CC will then communicate with the YMI Executive Director and appropriate contacts at the Mentor’s school(s).

  • Mentors with Symptoms – If a YMI Mentor develops symptoms while at their designated school(s), they are to leave and seek medical care or COVID-19 testing. A list of testing sites may be found at: www.fishersrecovery.com/testing/. Other available resources can be found via the ISDH website: www.coronavirus.in.gov/2524.htm.

  • Close Contact Procedure – If a YMI Mentor is notified that they may have been exposed via “close contact,” they are required to be tested for COVID-19 as well as quarantine for 14 days from the date of exposure, regardless of test result. 

  • Personal Protective Equipment – All YMI Mentors are required to wear a face mask while in their designated school(s). YMI will provide face masks for those who do not have their own. 

  • Social Distancing – To mitigate exposure, YMI Mentors are to abide by social distancing guidelines (stand / sit 6-feet away from their Mentee and school staff).

  • Personal Hygiene – YMI Mentors are to wash their hands or use other hand sanitizers frequently. It is recommended that reusable face mask(s) be washed frequently or use the disposable face masks provided by YMI.
