Rate yourself on a scale from 0 to 100 on how close you got to the goal of at least thirty minutes a day of physical activity that raised your heart rate. You should give yourself credit for any activity that raised your heart rate (brisk walking counts, a slow walk does not). For the purpose of this rating you don't get any extra credit for the intensity of the activity or activity that lasted more than 30 minutes in a day. This question requires a valid percent format.

Mindfulness practice refers to taking time out from planning or worrying and entering a state that is focused on awareness and acceptance but not attachment or worry. A good place to start is with some relaxing breaths and attention to your body, if you notice a worry let it pass through your mind like a bubble rising through a quiet lake. Mindfulness %
Regular morning social contact does not have to be more than a brief conversation. This question requires a valid percent format.
You get credit for conversations with someone in which you talked about how you were doing in an honest way. For instance, a conversation in which you might have shared your feelings, including any fears or concerns that you have had. 0 for none. 25 for one conversation. 50 for two conversations. This question requires a valid percent format.
Meaningful Social Interaction %
Proactivity is having a vision or goal for yourself. What do you want your life to be like? What do you want to achieve? You should rate yourself more based on taking steps every day (not big steps but regular steps) rather than on having big or detailed plans for your future. This question requires a valid percent format.
The goal here is to do something every day that is healthy and pleasurable. So, for example, watching TV usually doesn't count (it might be pleasurable but it isn't generally healthy), however you might already be achieving the goal if you are going for a walk somewhere that you enjoy in order to do your daily physical activity. Also, for folks who are depressed nothing might seem pleasurable, so in that case rate yourself as having achieved the goal if you do something that you would probably have enjoyed in the past. The key to this is noticing the things that you do well. For most people with depression it is noticing those things rather than doing them that is the challenge (yes you do things well!). This question requires a valid percent format.
This is a subjective rating. 0 to 10 for having little sense of meaning or purpose. 10 to 20 for some sense of meaning or purpose. 20 to 30 for a pretty clear sense of meaning or purpose. 30 to 40 for a strong sense of meaning or purpose. This question requires a valid percent format.
