K-12 COVID-19 Testing Program Participation Survey

The Wisconsin Department of Health Services (DHS) has received federal funding to develop a program to support school-based COVID-19 testing for teachers, staff, students, and their families. Schools and school districts should use this survey to indicate their interest in participating in the program. The survey is currently open. The survey is estimated to take 10 - 20 minutes to complete. Partial responses can be saved and returned to at a later date using the "Save and Continue Later" function at the top of each subsequent page.

INFORMATION REQUESTED: This survey will ask for information about your school or district's preferred testing types, models, schedules, and staffing needs. DHS staff will use this information to connect your school or district to available testing vendors or other resources to support your needs. All responses will be held confidential to the extent allowed by Wisconsin Open Records law.

SCHOOLS THAT ARE CURRENTLY TESTING: Schools that have received test supplies from DHS or partnered with an approved vendor to conduct testing during the spring and summer of 2021 must complete this survey to continue to access testing support for the 2021-2022 school year.
